What diabetic nephropathy is
Diabetic kidney disease is kidney damage caused by longstanding diabetes and is the leading cause of chronic kidney disease among adults with diabetes, affecting roughly one in three of them. Most patients have no symptoms until the disease is advanced, which is why routine screening matters.
Hyperfiltration comes first
The earliest change is glomerular hyperfiltration, an above normal glomerular filtration rate (GFR) driven by relative dilation of the afferent arteriole under chronic high blood glucose. This raises pressure inside the glomerular capillaries well before any structural damage is visible.
Structural progression to KW nodules
Persistent hyperglycemia, worsened by coexisting hypertension, drives diffuse expansion of the mesangial matrix and thickening of the glomerular basement membrane. Over years this can organize into rounded, periodic acid Schiff (PAS) positive hyaline masses called Kimmelstiel-Wilson (KW) nodules, the classic biopsy hallmark of advanced diabetic nephropathy.
